Register with the practice
We only accept patients who live in Swinton and the surrounding area. Once you have been accepted by the Practice you need to complete a registration form and return it to surgery along with a form of ID which can be one of the following:
- passport
- driving licence
- birth certificate
- utility bill (which must have current address on)
Patients are advised that they are registered with the Practice and you can see any GP of your preference.
When a patient registers with the practice they are given a “New Patient Pack” which will include a brief questionnaire to complete and are advised to make an appointment with a Practice Nurse for a short medical. This gives us a brief medical history and helps us until their records are received from their previous GP. The pack also includes all relevant information regards sharing of medical records and information about “Online Services”.

Temporary Residents
We are happy to see friends or relatives who are staying with our patients if they become ill. A temporary resident form needs to be completed with details of the home address and the home doctor.
