The staff at this practice record information about you and your health so that you can receive the right care and treatment. We need to record this information, together with the details of the care you receive because it may be needed if we see you again.

We may use some of this information for other reasons, for example, to help us to protect the health of the general public, to plan for the future.

Your Data Matters to the NHS

Patient Consent

Due the change in the Data Protection laws earlier this year if you need to discuss medical concerns on behalf of a family member or a person that you care for with a clinician then we need to obtain consent from the patient in order to do so. Your request will not be granted until we have written documentation confirming this agreement. A copy of the required form is available to download below, once complete this will need to be handed in at reception. Please contact the surgery for further information.

Patient Data

Crown Street Surgery ensures that all of our patient data is held within the strictest confidence. Our clinical systems can only be accessed by healthcare professionals, with a legitimate medical reason to view your information. We are fully compliant with the General Data Protection Regulation (2018)

Accessing Your Medical Records

Patients are able to access their personal information through a variety of means as detailed below:

Online Access

The easiest way to view your medical record is by using System Online which gives you the opportunity to view, and print your record. This is the fastest way in which a patient can access their information, and also the most secure because the online function is username and password controlled by you (the patient). Any patients wishing to view their personal information, but does not yet have access to System Online; can request an account by seeing a member of reception for more information.  Once you are registered and we have received the requested forms that have been signed, read and understood your request will then be considered by a GP.  When your request has been granted this will then allow you to view your detailed medical history information, from the date which your account is established. If you wish view previous medical information, then please let us know when you bring your System Online registration form in to the Practice, or speak to reception if you already use this service.

Subject Access Request (SAR)

Where a patient does not have access to view their medical record online, they can request a copy of the medical records. SAR’s can be submitted directly to the practice by using the form below, once this has brought into the surgery this will be dealt with within 30 days of your request.

Summarised Notes

As the NHS and technology have evolved, there has been a migration from using paper based records, to electronic records. In 2011 we undertook an extensive workload, to ensure that all patients’ paper records were summarised into their electronic record. Where a patient is requesting to view their medical records/history, we will always provide the electronic (summarised) version of their record. We kindly ask that any requests for information in its original no-summarised format are kept to a minimum and are requested only where absolutely necessary as this process is lengthy and very costly and incurs a considerable amount of administrative and GP time.

Access to Medical Reports Act (AMRA)

Any requests by or on behalf of individuals, to reports relating to themselves provided by medical practitioners for employment or insurance purposes and to make provision for related matters, must make a written request. As these constitute a chargeable non-NHS service, and are not covered within GDPR legislation, we require payment to be made up front, before any report or copy of examinations is released to the requester.

Chaperone Policy

If you feel your appointment requires a examination or procedure you can request a chaperone, please inform the receptionist.
